🩺 Utah Lets AI Renew Prescriptions

Utah has become the first U.S. state to allow an AI system to independently renew certain prescriptions. The pilot program, run with startup Doctronic, removes doctors from routine refill decisions. It marks a shift from AI advice to AI execution in healthcare. 

The Shift:

1. AI is now making bounded medical decisions - The system renews prescriptions across ~190 commonly used medications. Pain management drugs, ADHD treatments, and injectables are explicitly excluded for safety.

2. Safeguards are built into autonomy - In testing, AI decisions matched doctors ~99% of the time across 500 cases. Any uncertainty triggers escalation, and the first 250 renewals per drug class require human review.

3. Regulation remains unresolved - The FDA has not ruled on whether this counts as a regulated medical device. Federal involvement could determine how fast this spreads beyond Utah.

4. Expansion is already in motion - Doctronic charges $4 per refill and is in talks with Texas, Arizona, and Missouri. Leadership expects multiple states to follow in 2026.

This crosses a critical line from AI assisting care to AI delivering it. If prescription autonomy scales safely, it could lower costs and expand access, while forcing regulators and patients to decide how much medical authority machines should hold.
